Recreation Therapist

LifeBridge Health is seeking a Recreation Therapist to provide a continuum of assessment, planning, implementation, and evaluation of outcome-based treatment, leisure education, and recreation participation interventions for patients as ordered by the physician and/or designated by the manager.
Responsibilities:- Provide physician-ordered, billable treatment to a diverse population, including acute rehab, chronic care, ventilator-dependent, and Gero-Psych patients.
- Develop and implement individualized treatment plans to promote patient participation in leisure and recreational activities.
- Collaborate with interdisciplinary teams to ensure comprehensive care and coordination of services.
- Conduct assessments and evaluations to determine patient needs and progress.
- Document patient progress and outcomes in accordance with established policies and procedures.
- Certified Therapeutic Recreation Specialist (CTRS) or eligible.
- One year of experience or internship in a rehab/hospital setting.
- Experience with MDS and care planning process a plus.
