Clinical Liaison for Hospice Services RN Position
At Ohio's Hospice, Inc., we are looking for an empathetic and organized RN Admission Care Liaison to join our team in delivering high-quality patient care and coordination of hospice services. This is an excellent opportunity to apply your clinical expertise and interpersonal skills to make a meaningful impact on patients' lives.

About the Role:
As an RN Admission Care Liaison, you will be responsible for ensuring efficient patient admissions, providing education and resources to patients and their families, and maintaining accurate documentation in the electronic medical record. Your goal will be to achieve a conversion rate of 75% and utilize the electronic medical record skillfully. You will also need to maintain a valid driver's license with a safe driving record and provide transportation during daytime, nighttime, or inclement weather conditions.

Qualifications:
For this role, we require an active Registered Nurse license in the State of Ohio, two years of acute care nursing experience, and certification in Hospice & Palliative Medicine (CHPN) within two years of employment. Strong computer skills sufficient for proper documentation, proficiency in electronic medical records, and a deep understanding of Ohio's hospice regulations and policies are essential.

Compensation:
We offer a competitive salary of $80,000-$100,000 per year, depending on your experience.
