Job Summary

SAIL is seeking a highly skilled Substance Abuse Specialist to join our team. As a key member of our multidisciplinary team, you will provide direct clinical care to individuals with severe mental health symptoms and substance use disorders.

Key Responsibilities:

  • Provide direct, face-to-face services and treatment to individuals in the community.
  • Develop and implement person-centered goals with clients to foster quality of care.
  • Provide counseling, supportive services, and strategies to maintain client safety and wellness.
  • Develop treatment and recovery plans and identify behaviors that impede rehabilitation.
  • Provide trauma-informed care and reflect and incorporate clients' personal narratives.
  • Monitor, evaluate, and record clients' progress and modify treatment regimens as needed.
  • Build relationships with clients and their families or support systems.
  • Work with clients and their families or support systems in transitions of care and ensure stable links and resources at discharge.
  • Ensure daily and frequent communication to promote active collaboration.
  • Attend daily team meetings to ensure the continuum of care.
  • Use a shared decision-making process and work collaboratively with other mental health care professionals.
  • Provide ongoing education and training to team members.
  • Support team members through open communication.
  • May facilitate individual and group educational sessions.
  • Observe the Rights of Recipients as defined in the NYSOMH ACT Program Guidelines.
  • Successfully complete training modules in the Center for Practice Innovation's Learning Management System (CPI) for ACT Core training.

Requirements:

  • Master's degree in human services accompanied by appropriate certification or license.
  • Three years' experience in a human service-related field, with experience on a multi-disciplinary mobile team preferred.
  • One year experience in specialty area preferred.
  • Demonstrates organizational and communication skills, critical thinking, and sound judgment.
  • Ability to work independently and as part of a team.

Physical Demands and Working Conditions:

  • Transports clients in personal or agency vehicle as needed.
  • Travels to client sites, Agency sites, community sites, offsite entities, and/or other office locations as needed.
  • Participates in a team approach and multi-disciplinary treatment.
  • Work staggering schedules between the hours of operations of 8am-8pm.
  • Provides on-call duties after normal business hours as per the rotation schedule.
  • Requires direct in-person/face-to-face client interaction.
  • Ability to work amongst diverse social, cultural, and economic groups.
