Interim HealthCare Hospice is seeking a compassionate and skilled Medical Social Worker to join our team. As a Medical Social Worker, you will play a vital role in providing emotional, psychological, and social support to our patients and their families.
Key Responsibilities:
- Assess the ongoing psychosocial status of patients and families to identify their needs and develop a plan of care.
- Provide counseling and support to patients and families to help them cope with the terminal illness and related issues.
- Collaborate with the Interdisciplinary Group (IDG) to develop and implement a comprehensive plan of care that meets the patient's physical, emotional, and social needs.
- Document patient progress and maintain accurate records of patient interactions and care plans.
- Communicate effectively with patients, families, and healthcare team members to ensure seamless care coordination.
- Participate in ongoing education and training to stay current with best practices in hospice care and social work.
Requirements:
- Master's degree in Social Work from a Council on Social Work Education (CSWE) accredited program.
- Active licensure as a Social Worker in the state of California.
- Minimum of one year of experience in healthcare, preferably in hospice care.
- Knowledge of hospice philosophy and commitment to providing patient-centered care.
- Excellent communication, interpersonal, and organizational skills.
- Ability to work effectively in a team environment and prioritize multiple tasks and responsibilities.
