Nurse Care Coordinator

We are seeking a compassionate and skilled Registered Nurse to join our team as a Nurse Care Coordinator. In this role, you will provide high-quality patient care and coordinate services to meet the needs of individuals and families in their homes and communities.
Responsibilities- Conduct comprehensive assessments of patients and families to determine their needs and develop individualized care plans.
- Provide education and support to patients and families on disease management, medication adherence, and self-care techniques.
- Collaborate with the interdisciplinary team to ensure seamless care coordination and communication.
- Document patient progress and outcomes in a timely and accurate manner.
- Participate in quality improvement initiatives to enhance patient care and outcomes.
- Graduate of an accredited school of nursing with a current RN license in the state.
- Minimum one year of experience in hospice or home health care.
- Excellent communication and interpersonal skills.
- Ability to work independently and as part of a team.
