Job Title: Home Health Physical Therapy Assistant

Alternate Solutions Health Network LLC is seeking a skilled Physical Therapist Assistant to join our team in Covington, Kentucky.

Job Summary:

  • Provide comprehensive skilled services as ordered by the attending physician.
  • Assess patient conditions and notify the Physical Therapist of any changes.
  • Treat patients to relieve pain, develop or restore function, and maintain maximum performance.
  • Minimize patient risk by assessing functional problems and providing intervention.
  • Counsel patients and families on disease/injury management and proper use of equipment.

Responsibilities:

  • Plan of Care: Develop and implement individualized plans of care for patients.
  • Patient Outcomes: Treat patients to achieve optimal outcomes and improve functional status.
  • Minimize Patient Risk: Identify and mitigate risks to patient safety and well-being.
  • Family Educator/Advocate: Educate patients and families on disease management and equipment use.
  • Policies: Maintain accurate and timely documentation and adhere to agency policies and procedures.

Requirements:

  • Current Physical Therapist Assistant (PTA) license in the state of employment.
  • Minimum one-year experience as a PTA in an acute care setting.
  • Valid driver's license and auto insurance.
