Job Summary:

  • Provide comprehensive skilled services as ordered by the attending physician.
  • Develop and implement individualized plans of care to achieve optimal patient outcomes.
  • Collaborate with interdisciplinary teams to ensure seamless care coordination.

Responsibilities:

  • Plan of Care: Develop and implement individualized plans of care to achieve optimal patient outcomes.
  • Patient Outcomes: Treat patients to relieve pain, develop or restore function, and maintain maximum performance using physical means.
  • Minimize Patient Risk: Help decrease re-hospitalizations by front-loading visits for high-risk patients and teaching preventative measures.
  • Family Educator/Advocate: Counsel patients and families on disease/injury management and instruct on proper use of equipment.
  • Supervision: Oversee and supervise therapy services provided by PTAs and nurse assistants.
  • Communication/Collaboration: Communicate with PTAs and nurse aides on supervisory visit results.
  • Policies: Complete all clinical documentation following agency protocol and Medicare/Federal guidelines.
  • Rules and Regulations: Understand and follow agency policies, procedures, rules, and regulations.

Requirements:

  • Current license as a Physical Therapist in the state of employment.
  • Valid driver's license and auto insurance in your name as a driver.
  • Capable of all physical demands.
