Key Responsibilities- Develop and implement individualized plans of care for patients, focusing on pain relief, functional restoration, and maximizing performance.
- Conduct thorough assessments, record patient reactions, and report progress to attending physicians and relevant agency staff.
- Perform OASIS assessments to create and adjust patient care plans as needed.
- Collaborate with interdisciplinary teams to ensure comprehensive patient care and minimize risk of re-hospitalization.
- Provide education and support to patients and their families on disease management, prevention, and proper use of equipment.
- Supervise and oversee therapy services provided by Physical Therapist Assistants and nurse assistants when necessary.
- Communicate effectively with patients, families, physicians, and colleagues to ensure seamless care coordination.
- Maintain accurate and timely documentation, adhering to agency protocols and Medicare/Federal guidelines.
- Current Physical Therapist license in the state of employment.
- Valid driver's license and auto insurance in your name as a driver.
- Ability to meet all physical demands of the job.
