Clinical Patient Care Manager

The Clinical Patient Care Manager is responsible for leading the Patient Care Team and ensuring the delivery of high-quality, patient-centered care. This includes:
- Directing all patient care services, including volunteer and bereavement support, to ensure seamless integration with the interdisciplinary plan of care.
- Reviewing patient information to ensure eligibility and appropriateness criteria are met.
- Collaborating with the team physician and attending physician to develop palliative care plans for pain and symptom management.
- Ensuring accurate and complete data and documentation are available for recertification decisions.
- Reviewing imminent discharges to ensure effective discharge planning.
- Monitoring team services to ensure they are responsive to patient and family needs.
- Managing the team schedule and coordinating services 24/7 to meet patient needs.
- Facilitating team meetings to ensure interdisciplinary input into the plan of care.
The Clinical Patient Care Manager is responsible for supervising and managing the Patient Care Team, including:
- Interviewing, selecting, training, and evaluating team staff in conjunction with the Patient Care Administrator.
- Ensuring staff competence and performance levels through field visits, documentation review, and in-service training.
- Acting as a resource and mentor for staff on clinical issues, documentation, and customer service.
- Overseeing staff and volunteer schedules to ensure equitable workload distribution.
- Reviewing and approving payroll to ensure accuracy.
The Clinical Patient Care Manager is responsible for ensuring quality improvement and regulatory compliance, including:
- Monitoring clinical records and team activities to ensure accuracy and completeness.
- Developing and implementing performance improvement activities to address service issues.
- Ensuring team completion of patient and family satisfaction surveys and implementing quality improvement measures.
- Assisting with surveys and requests from regulatory bodies.
- Performing substantive chart reviews to ensure quality care delivery.
- Participating in outcomes management and annual program review.
The Clinical Patient Care Manager is responsible for ensuring customer satisfaction and sales/marketing efforts, including:
- Addressing patient and family concerns and resolving service failures promptly.
- Analyzing customer service issues to identify causes and improve performance.
- Visiting LTC and Contract Bed facilities to ensure care plan integration and customer satisfaction.
- Participating in inservices and professional service organizations.
The Clinical Patient Care Manager is responsible for ensuring cost containment, including:
- Verifying and approving admitting diagnoses, treatments, staffing, and supplies.
- Monitoring resource utilization to ensure cost-effective delivery of services.
- Controlling and being accountable for productivity, labor, and patient care costs.
- Approving all bills and invoices related to patient care services.
The Clinical Patient Care Manager is responsible for ensuring professional development, including:
- Attending inservices, educational seminars, and workshops.
- Developing and achieving professional growth goals and objectives.
- Participating as a mentor for newly hired Team Managers.
Qualifications:
- Reliable transportation with appropriate license and insurance coverage.
- Private telephone in home.
- Knowledge of primary medical and nursing care principles, with a working knowledge of oncologic nursing and palliative care.
- Basic understanding of interpersonal dynamics within interdisciplinary healthcare teams and knowledge of the process of dying and bereavement.
- Working knowledge of VITAS information system (Vx) and management reports.
- Two years successful supervisory experience or equivalent in a healthcare organization.
Education:
- Bachelor's degree preferred.
- Current and valid R.N. License to practice in the state where the VITAS program is located.
