About the Role

We are seeking a highly skilled LPN Medication Care Manager to join our team at Sunrise Senior Living. As a Medication Care Manager, you will be responsible for providing the highest level of quality care and services to our residents by administering medication and treatments in a safe and organized manner.

Key Responsibilities
  • Administer medications and treatments to residents in accordance with their individualized care plans.
  • Conduct daily medication reviews and updates to ensure accurate and timely administration of medications.
  • Document all medication administration, including any refusals or missed doses.
  • Collaborate with the Resident Care Director and Wellness Nurse to ensure accurate and timely medication administration.
  • Participate in the development and implementation of Individualized Service Plans (ISPs) and Individualized Care Plans (ICPs).
  • Conduct regular audits of medication carts and supplies to ensure compliance with regulations and Sunrise policies.
Requirements
  • Licensed Practical Nurse (LPN) or Licensed Vocational Nurse (LVN) credential.
  • Previous experience working with seniors and in a medication management role.
  • Strong communication and interpersonal skills.
  • Ability to work independently and as part of a team.
  • Ability to maintain confidentiality and handle sensitive information.
  • Knowledge of medication administration protocols and regulations.
