Licensed Practical Nurse

We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our team at Bridgewell, a leading provider of social and human services. As an LPN, you will play a vital role in delivering high-quality healthcare services to individuals with disabilities living in our residential program.
Key Responsibilities:- Provide direct nursing care to individuals with disabilities, including medication administration, wound care, and vital sign monitoring.
- Collaborate with the residential manager, registered nurse, and Director of Health Care to ensure seamless care coordination.
- Act as a liaison with community health care providers, monitoring medical records and advocating for individuals' needs.
- Document health care services accurately and maintain confidentiality.
- Participate in ongoing education and training to enhance skills and knowledge.
- Graduation from an accredited LPN program.
- Current Massachusetts Nursing license.
- Two years of general nursing experience and/or service to individuals with developmental disabilities in a community health setting (preferred).
- Ability to foster positive relationships and communicate effectively with individuals, families, and healthcare teams.
