About the Role

Rio Grande Regional Hospital is seeking a skilled Registered Nurse Case Manager PRN to join our team. As a key member of our healthcare team, you will play a vital role in ensuring that patients receive high-quality, efficient care.

Key Responsibilities

  • Facilitate care along a continuum through effective resource coordination to help patients achieve optimal health, access to care, and appropriate utilization of resources.
  • Assess patients for transition needs to promote timely throughput, safe discharge, and prevent avoidable readmissions.
  • Collaborate with healthcare teams to develop and implement individualized care plans that address patients' physical, psychosocial, and vocational needs.
  • Monitor patients' progress and adjust care plans as needed to ensure optimal outcomes.

Requirements

  • Associate Degree in Nursing, required
  • Bachelor's Degree in Nursing, preferred
  • Case Management certification, preferred
  • Current Texas licensure with State Board of Nurse Examiners as an RN
  • 1+ years experience in healthcare
