As a Skilled Home Health RN Case Manager, you will have the opportunity to make a meaningful impact in the lives of our patients and their families.
Responsibilities:
Position Summary:
As a Skilled Home Health RN Case Manager, you will provide highly skilled nursing care in the home setting, supervising LVN care delivery and coordinating with the multidisciplinary team to ensure seamless care.
You will be the front-line clinician, communicating with physicians and overseeing the plan of care to improve clinical outcomes.

Qualifications:
Minimum:
California Registered Nurse license
Current AHA BLS certification
California drivers license with proof of liability insurance for automobile
Preferred:
One year acute care experience as a RN
Home Health experience preferred
