Responsibilities:

  • Provide highly skilled nursing care in the home setting as the admitting nurse.
  • Supervise LVN care delivery and care coordination with the multidisciplinary team.
  • Communicate with physicians to oversee the plan of care.
  • Lead the case conference and improve clinical outcomes.

Qualifications:

  • California Registered Nurse license.
  • Current AHA BLS certification.
  • California drivers license with proof of liability insurance for automobile.

Preferred Qualifications:

  • One year acute care experience as a RN.
  • Home Health experience preferred.
