As a Case Manager, you will provide highly skilled nursing care in the home setting, overseeing the plan of care and communicating with physicians to ensure the best possible outcomes for our patients.

Responsibilities:

  • Provide wound care, IV medication administration, and supervise LVN care delivery
  • Coordinate care with the multidisciplinary team to ensure seamless patient care
  • Communicate with physicians to ensure accurate and timely care
  • Lead case conferences to improve clinical outcomes

Requirements:

  • California Registered Nurse license
  • Current AHA BLS certification
  • California driver's license with proof of liability insurance for automobile

Preferred Qualifications:

  • One year acute care experience as a RN
  • Home Health experience preferred
