Job Summary

Riverside Health System is seeking a skilled Licensed Practical Nurse (LPN) to join our team in Williamsburg, Virginia. As an LPN, you will play a vital role in providing high-quality patient care and contributing to the success of our medical practice.

Key Responsibilities
  • Conduct focused assessments of patients to include vital signs and gather data
  • Implement established plans of care and evaluate the effectiveness of treatments
  • Accurately complete all required clinical documentation consistently per RHS standards
  • Maintain documentation for quality assurance, including point of care testing, medication logs, and annual wellness visits
  • Work with providers to address care gaps and ensure accurate, legible, and timely documentation
  • Provide patient education and teach patients about their medications
  • Communicate lab/pathology results and schedule additional tests as needed
  • Ensure proper reimbursement from insurance companies by performing pre-authorization for prescription drugs, procedures, and patient tests
  • Demonstrate the ability to build trusting collaborative relationships with a multi-disciplinary team to support patient care needs
Requirements
  • Program Graduate, Licensed Practical Nurse (Preferred)
  • Bachelors Degree, Nursing (Preferred)
  • 1 year Nursing experience (Preferred)
  • Licensed Practical Nurse (LPN) - Virginia Department of Health Professions (VDHP) Upon Hire (Required)
  • CPR/BLS Certification - American Heart Association/American Red Cross/American Safety and Health Institute (AHA/ARC) Upon Hire (Required)
