Job Description

At GEO Reentry Services, LLC, we are seeking a highly motivated and experienced Case Manager to join our team. As a Case Manager, you will play a critical role in supporting the rehabilitation and reintegration of individuals into their communities.

Responsibilities
  • Manage a caseload of participants, providing individualized support and guidance to help them achieve their rehabilitation goals.
  • Conduct risk/need assessments and create treatment plans to address participants' specific needs and challenges.
  • Meet regularly with participants to monitor their progress, provide feedback, and make adjustments to their treatment plans as needed.
  • Collaborate with other stakeholders, including law enforcement, social services, and community organizations, to ensure a comprehensive and coordinated approach to rehabilitation and reentry.
  • Develop and implement evidence-based programs and services to support participants' rehabilitation and reintegration into their communities.
  • Provide education and training to participants on topics such as life skills, employment, and housing.
  • Monitor and report on participants' progress, including any challenges or successes, to ensure that they are meeting their rehabilitation goals.
  • Develop and maintain relationships with community partners and stakeholders to ensure a strong network of support for participants.
Requirements
  • Bachelor's degree in a related field, such as social work, counseling, or criminal justice.
  • Minimum of 2 years of experience in a related field, such as case management, counseling, or social work.
  • Strong knowledge of the criminal justice system and rehabilitation principles.
  • Excellent communication and interpersonal skills, with the ability to work effectively with diverse populations.
  • Ability to work in a fast-paced environment and prioritize multiple tasks and responsibilities.
  • Valid driver's license and reliable transportation.
