About the Role

The Case Manager (CM) plays a vital role in ensuring high-quality, patient-centered care by managing rehabilitative care to achieve optimum outcomes. This involves coordinating efforts within the rehabilitation team, collaborating with the Rehab Program Director and Facility Case Management Director, and ensuring the implementation of the patient's treatment plan that supports their strengths, abilities, needs, and preferences.

Responsibilities
  • Facilitate the involvement of the patient throughout the rehabilitation process
  • Document the findings of the Discharge Planning Evaluation (DPE) and psychosocial assessments
  • Communicate the social, financial, or discharge needs and preferences of the patient/family/caregiver
  • Ensure the implementation of the patient's treatment plan that supports their strengths, abilities, needs, and preferences
Requirements

We require an Associate Degree or Bachelors Degree in Nursing, with 2+ years of experience in case management or 3+ years of experience in clinical nursing.
