Chicago Illinois, United States Wellbe Senior Medical Full time

Position Overview:

ABOUT WELLBE SENIOR MEDICAL

At WellBe, we are dedicated to transforming the delivery of healthcare across the nation. Our mission is to provide home-based medical services to those who need it most, ensuring that care is accessible wherever our patients reside.

Our primary care is administered by a team of licensed healthcare professionals who visit patients in the comfort of their homes.

By integrating the expertise of physicians, nurse practitioners, social workers, paramedics, and other healthcare specialists, WellBe offers an enhanced level of support to the communities we serve.

This demographic often faces barriers to accessing care, which we aim to overcome by bringing healthcare directly to them.

WellBe's clinician-led geriatric care teams deliver personalized medical attention and social support at home, while also coordinating care across the entire healthcare spectrum – from chronic and urgent care to hospital services, skilled nursing facilities, assisted living, palliative care, and end-of-life support.

Joining WellBe means becoming part of a growing, mission-driven organization committed to providing exceptional care to senior communities and positively impacting lives every day.
