Hospice Clinical Liaison Job Summary

The Hospice Clinical Liaison serves as the first point of contact for hospitalized patients and their families, introducing them to hospice and palliative care. This role educates patients and their families on hospice care and provides ongoing support.

Key Responsibilities

Initiates the plan of care, discharge plan, and communicates with the interdisciplinary team regarding patient goals and treatment plans.
Makes nursing assessments, judgments, and treatment decisions based on patient needs and wishes.
Promotes Capital Caring and Capital Palliative Care Consultants programs to acute care facilities through collaborative relationships with the facility by providing opportunities for professional development and community education.

Qualifications

Must have one year of experience in a comparable setting/unit. Hospice and/or palliative care experience is strongly desired.
Must have a degree in Nursing from an accredited nursing program. A BSN degree is desired.
Must have licensure from the governing body for practice for the District of Columbia, State of Maryland, and/or the Commonwealth of Virginia, based on the state/jurisdiction of practice. Licensure from compact state is acceptable.
Must have a driver's license in good standing and access to a vehicle during working hours.

Working Conditions

May be asked to rotate to other regions/departments. COVID-19 vaccine required to be completed upon start.
