Key Responsibilities:
  • Conducts thorough assessments and develops tailored care plans in collaboration with members, families, and healthcare providers.
  • Evaluates living conditions, cultural influences, and individual functioning to identify specific needs and create comprehensive care strategies.
  • Determines eligibility for program services based on health, medical, financial, legal, and psychosocial factors, both initially and continuously.
  • Establishes actionable, time-sensitive objectives and goals to meet identified member needs.
  • Implements targeted care management interventions to achieve the established goals.
  • Coordinates long-term care services across various settings, including home and community-based environments.
  • Engages with interdisciplinary teams, healthcare providers, and family members to ensure optimal service delivery and coordination.
  • Monitors care management activities and evaluates the effectiveness of interventions to ensure desired outcomes.
  • Identifies community trends and needs, planning interventions accordingly.
  • Documents all services in compliance with community care standards and regulatory requirements.

Qualifications:

Licenses and Certifications:

  • Preferred: Licensed Social Worker in New York State.


Education:

  • Master's Degree in Social Work is required.
  • Case Management Certification is preferred.


Experience:

  • A minimum of three years of experience in Social Work is required.
  • At least two years in a case management or community-based setting is preferred.
  • Bilingual skills may be necessary based on operational requirements.
  • Preferred: Clinical expertise in geriatrics and experience in managed care.
