Job Title: Licensed Practical Nurse

We are seeking a skilled and compassionate Licensed Practical Nurse to join our team at Episcopal SeniorLife Communities. As a Licensed Practical Nurse, you will play a vital role in providing high-quality care to our residents, working collaboratively with our interdisciplinary team to ensure the best possible outcomes.

Responsibilities:
  • Provide direct patient care, including administering medications, performing treatments, and monitoring vital signs.
  • Develop and implement individualized care plans, taking into account the unique needs and goals of each resident.
  • Collaborate with other healthcare professionals to ensure seamless care transitions and optimal patient outcomes.
  • Communicate effectively with residents, families, and healthcare teams to ensure informed decision-making and care coordination.
  • Participate in quality improvement initiatives and contribute to the development of policies and procedures.
Requirements:
  • Current NYS practical nurse licensure.
  • Minimum one year clinical experience, preferably in gerontology.
  • Effective written and verbal communication skills.
  • Ability to interact courteously and tactfully with staff, residents, family members, visitors, vendors, and the general public.
  • Adult CPR Certified from the American Heart Association or Red Cross.
