Key Responsibilities- Assess and develop individualized care plans that address the unique needs and goals of each person
- Coordinate with healthcare providers, community resources, and family members to ensure seamless care transitions
- Identify and connect individuals with community resources, social services, and other support systems
- Monitor and evaluate the effectiveness of care plans, making adjustments as needed
- Communicate effectively with all stakeholders, including individuals, families, and healthcare providers
- Bachelor's degree in social work, rehabilitation, nursing, psychology, or related field
- 2+ years of experience working in a healthcare or social services setting
- 1+ year of experience working with individuals with long-term care needs
- Strong communication and interpersonal skills
- Ability to work independently and as part of a team
- Licensed Social Worker or clinical degree
- Experience working with electronic health records and other healthcare software
- Knowledge of community resources and social services
