We are seeking a skilled Care Coordinator to join our team at UnitedHealthcare. As a Care Coordinator, you will play a critical role in ensuring that individuals receiving Long-Term Services and Supports (LTSS) receive high-quality, person-centered care.

Key Responsibilities
  • Assess and develop individualized care plans that address the unique needs and goals of each person
  • Coordinate with healthcare providers, community resources, and family members to ensure seamless care transitions
  • Identify and connect individuals with community resources, social services, and other support systems
  • Monitor and evaluate the effectiveness of care plans, making adjustments as needed
  • Communicate effectively with all stakeholders, including individuals, families, and healthcare providers
Requirements
  • Bachelor's degree in social work, rehabilitation, nursing, psychology, or related field
  • 2+ years of experience working in a healthcare or social services setting
  • 1+ year of experience working with individuals with long-term care needs
  • Strong communication and interpersonal skills
  • Ability to work independently and as part of a team
Preferred Qualifications
  • Licensed Social Worker or clinical degree
  • Experience working with electronic health records and other healthcare software
  • Knowledge of community resources and social services
