Job Summary:
The Care Coordinator Nurse is a vital member of our healthcare team at Dayton Children's, responsible for ensuring the provision of high-quality patient care in the appropriate setting. This role involves care coordination, case management, and transitions of care to different levels of care, in collaboration with the attending practitioner, family, and other members of the healthcare team.
Key Responsibilities:
- Coordinate individualized plans of care and communicate the plan to appropriate stakeholders.
- Ensure high-quality care is provided as efficiently and cost-effectively as possible.
- Facilitate continuity of care by coordinating transitions to post-discharge care.
- Practice professional nursing and promote patient advocacy in accordance with the ANA Code of Ethics for Nurses.
Department Specific Requirements:
- Full-time, 40 hours per week.
- Monday to Friday, pm schedule.
- No weekends or holidays.
- Three years of nursing experience required.
Education and Certification Requirements:
- Bachelor's degree in Nursing (required).
- Registered Nurse (RN) licensure in Ohio (required).
- CPR certification (required).
