Job Description

The Nightshift Care Coordinator is a critical role at Carlyle House Assisted Living Community, responsible for overseeing the management, development, and performance of our Care Advocates personnel.

Key Responsibilities
  • Manage and develop the overall quality of care services provided by Carlyle House, ensuring compliance with state and federal regulations.
  • Direct the care needs of residents according to their care plans, communicating effectively with the Nursing Department and other stakeholders.
  • Conduct regular staff meetings to plan, coordinate, and implement direct care service policies and procedures.
  • Communicate clearly and concisely with Care Advocates, providing feedback and guidance as needed.
  • Work closely with the Health Care Director and Assistant Director of Nursing to review reports of accidents/incidents and take appropriate disciplinary action.
  • Assist with investigations involving allegations of abuse, neglect, or misappropriation of resident property.
  • Maintain up-to-date position control and master schedules, ensuring adequate staffing levels.
  • Develop and deliver training programs for new and existing Care Advocates, focusing on best practices and regulatory compliance.
  • Supervise Care Advocates during normal shifts and meetings, conducting daily rounds to ensure residents' ADLs are met.
  • Communicate any changes in resident condition or care needs to the Nursing Department and other relevant stakeholders.
  • Ensure staff awareness of all state and federal regulations, including OSHA.
  • Responsible for corrective action and follow-up, ensuring compliance with company policies and procedures.
Requirements
  • Good physical and mental health, with ability to lift, carry, and move continuously during work hours.
  • Ability to work a flexible schedule, including nights and weekends.
  • Strong communication and leadership skills, with ability to motivate and develop Care Advocates.
  • Knowledge of state and federal regulations, including OSHA.
