About the Role

At Alta Hospitals, we are seeking a skilled and compassionate Outpatient Care Coordinator II to join our team. This role is responsible for identifying and reducing barriers to members' care, assisting in accessing resources, and educating on the importance of timely care.

Responsibilities
  • Provide support to members as they navigate the healthcare system, ensuring seamless coordination of care.
  • Work closely with case management teams to resolve access, medical, and psychosocial related issues, promoting optimal member outcomes.
  • Efficiently manage and prioritize assigned caseloads, leveraging effective time management skills to meet productivity guidelines.
  • Collaborate with licensed clinical professionals to ensure optimal coordination of care, fostering a culture of teamwork and communication.
  • Serve as a liaison between Health Plan members, primary care providers, specialists, and other healthcare team members, facilitating warm transfers and referrals to community resources.
What We Offer

As an Outpatient Care Coordinator II at Alta Hospitals, you can expect a competitive salary range of $65,000 - $85,000 per year, commensurate with experience.
