Job Summary

We are seeking a skilled Licensed Practical Nurse (LPN) to join our team at VNA of Cape Cod. As a LPN, you will play a vital role in providing high-quality patient care and supporting our patients and families in their homes.

Key Responsibilities
  • Assess patient needs and report new or pertinent findings to the Registered Nurse (RN) and Medical Doctor (MD)
  • Implement the medical plan of care, nursing interventions, and procedures as necessary for patient care, within the guidelines of licensure
  • Evaluate and reassess patient needs, care provided, and patient response on a continuing basis
  • Identify ancillary services needed by patients and families and report findings to the RN
  • Collaborate and communicate with the Team Leader, Office Nurse, RN, and inter-disciplinary team members as needed
  • Assist patients and families in accessing community resources
  • Record narrative documentation to reflect skilled care, ensuring clear, concise, and specific documentation of teaching, direct care, and patient/caregiver response
Requirements
  • Graduate of an approved LPN school of nursing
  • Current LPN license in Massachusetts
  • CPR certification required
  • Minimum one (1) year recent experience in an acute care facility
  • Homecare and computer skills preferred
