Job Summary

We are seeking a skilled and compassionate Licensed Practical Nurse (LPN) to join our team at East Longmeadow Skilled Nursing Center. As an LPN, you will play a vital role in providing high-quality patient care and contributing to the overall well-being of our residents.

Key Responsibilities
  • Provide basic nursing care, including vital sign checks, wound care, and medication administration.
  • Document patient care and progress, ensuring accurate and timely reporting to the healthcare team.
  • Communicate effectively with patients, families, and healthcare professionals to ensure seamless care coordination.
  • Collaborate with the interdisciplinary team to develop and implement individualized care plans.
  • Administer medications and treatments as prescribed by physicians, ensuring patient safety and comfort.
  • Participate in emergency situations, responding promptly and professionally to ensure patient well-being.
  • Perform administrative tasks, such as charting, reporting, and completing medical forms.
Requirements

To be successful in this role, you will need:

  • A current LPN license in the state of Massachusetts.
  • Excellent communication and interpersonal skills.
  • Ability to work effectively in a fast-paced environment.
  • Strong attention to detail and organizational skills.
  • Ability to lift, move, and position patients as needed.
