Job Summary

We are seeking a skilled and compassionate Licensed Practical Nurse (LPN) to join our team at East Longmeadow Skilled Nursing Center. As an LPN, you will play a vital role in providing high-quality patient care and contributing to the overall well-being of our residents.

Key Responsibilities
  • Provide basic nursing care, including vital sign checks, wound care, and medication administration.
  • Document patient care and progress, ensuring accurate and timely reporting.
  • Communicate effectively with RNs, physicians, and other healthcare team members to ensure seamless care coordination.
  • Collaborate with colleagues to promote a positive and supportive work environment.
  • Teach patients and their families about health information and provide education on disease management and prevention.
  • Administer medications and treatments as prescribed, ensuring patient safety and comfort.
  • Respond to emergencies and provide prompt, professional care.
  • Admit, transfer, and discharge residents as required, ensuring smooth transitions and minimal disruption to care.
  • Perform administrative tasks, including charting, reporting, and completing medical forms.
  • Verify patient identities and ensure accurate medication administration.
  • Maintain a safe and clean environment, adhering to infection control policies and procedures.
Requirements

To be successful in this role, you will need:

  • A current LPN license in the state of Massachusetts.
  • Excellent communication and interpersonal skills.
  • Ability to work effectively in a fast-paced environment.
  • Strong attention to detail and organizational skills.
  • Ability to lift, move, and position patients as needed.
