About the Job
We are seeking a Clinical Nurse Leader PRN to join our team. This role will involve coordinating patient care, managing length of stay, and ensuring appropriate resource utilization. The ideal candidate will have 3+ years of clinical hospital nursing experience and 2 years of Case Management experience.

Responsibilities
- Conduct comprehensive assessments of patients' medical, psychosocial, and discharge needs
- Develop and implement individualized plans of care, in collaboration with the multidisciplinary team
- Coordinate activities that promote quality outcomes and patient throughput
- Collaborate with Social Services to ensure post-acute medical needs and level of care are appropriate
- Refer patients to third-party payer, disease, and case management programs as needed
- Facilitate effective care transitions, ensuring seamless handoffs between teams
- Document professional recommendations, discharge plans, and care coordination interventions

Requirements
- Registered Nurse with current TX state license required
- Associate's degree in nursing required; Bachelor's degree preferred
- 3+ years of clinical hospital nursing experience required
- 2 years of Case Management experience required
- Certification in Case Management preferred

Benefits
Medical City Plano offers a competitive salary range of $83,465 - $144,833 per year, depending on experience. We also offer a comprehensive benefits package, including health insurance, 401(k) matching, and paid time off.
