Position Overview

The Healthcare Services Coordination Specialist is essential in facilitating healthcare delivery for patients within the Selikoff Centers for Occupational Health and the WTC Health Program Clinical Center of Excellence at Mount Sinai. This role acts as a bridge for patient care activities, ensuring seamless coordination throughout the patient’s journey and subsequent interactions.

Key Responsibilities

  • Lead the navigation of patient care, ensuring patients comprehend their program benefits and what to expect from the services provided.
  • Collaborate with patients and care teams to establish care goals, documenting these in the program dashboard and calendar tool. Follow-up tracking includes visit reminders, scheduled follow-ups, authorization submissions, and coordination of specialty appointments.
  • Confirm that all provider orders are authorized, ensuring services align with the patient's covered benefits.
  • Serve as the primary contact for educating patients about their pharmacy benefits and assist in obtaining prior authorizations as necessary.
  • Organize patient communications and future program plans based on interactions recorded in the electronic medical record and program benefits systems, ensuring timely documentation and follow-up on required action items.
  • Provide extensive patient navigation between program visits, addressing concerns, facilitating discussions, and coordinating with health plan analysts and case managers to enhance patient care.
  • Educate patients about the full spectrum of benefits offered by the program, including the certification process and various compensation funds.
  • Propose recommendations for process enhancements aimed at improving patient experiences and participate in pilot programs to boost patient engagement.
  • Perform additional duties as assigned.

Qualifications

  • High School Diploma or equivalent; Bachelor's Degree preferred or an equivalent combination of education and experience.
  • A minimum of 3 years of experience in healthcare or customer service roles.
