About the Role:
The Case Manager - Social Work Specialist at Good Samaritan Hospital is responsible for providing comprehensive social work services to patients and families to identify and resolve problems that impede discharge planning or the patient's use of healthcare resources.
Key Responsibilities:
- Assess and evaluate patients to assist with discharge planning, ensuring a smooth transition to post-acute care.
- Develop and implement individualized care plans, taking into account the patient's medical, emotional, and social needs.
- Collaborate with multidisciplinary teams, including physicians, nurses, and other healthcare professionals, to promote care coordination and ensure seamless patient care.
- Provide emotional support and counseling to patients and families, addressing concerns related to illness, disability, and healthcare navigation.
- Identify and mobilize community resources to meet the needs of patients and families, ensuring access to necessary services and support.
- Document all interventions and services provided, maintaining accurate and timely records.
- Serve as a resource to staff, providing guidance and support on complex cases and social work best practices.
- Organize and prioritize caseload, ensuring efficient and effective management of patient care.
Requirements:
- Master's Degree in Social Work from an accredited School of Social Work.
- One year of experience in a healthcare setting, preferably in a hospital environment.
- Valid license and current registration as a Licensed Master Social Worker (LMSW) or Licensed Clinical Social Worker (LCSW) issued by the New York State Education Department.
