Job Title: Licensed Practical Nurse

At The Resource Center, we are seeking a highly skilled and compassionate Licensed Practical Nurse (LPN) to join our team. As an LPN, you will play a vital role in providing high-quality healthcare services to individuals with disabilities in our certified residences.

Responsibilities:
  • Provide direct care and support services to residents, including personal care, daily living, and community activities.
  • Administer medications and treatments as ordered by a physician or nurse practitioner.
  • Monitor the health status of individuals living in assigned residences and provide training to direct support professionals on general health-related issues.
  • Document pertinent contacts made on behalf of individuals and any new medical/health-related information.
  • Ensure accurate documentation of vital signs, weight, intake/output, seizure activity, response to medication/treatment for individuals assigned to their care.
  • Communicate effectively with RNs, residential staff, and other service providers to ensure continuity of health services and timely identification of health concerns.
Requirements:
  • New York State licensure or eligibility to practice as a Licensed Practical Nurse.
  • Professional experience preferred, with experience working with individuals with disabilities a plus.
  • Valid driver's license and acceptable driving record, according to agency insurance carrier guidelines.
  • Current certification in Cardiopulmonary Resuscitation (CPR) and First Aid.
