Social Worker for Palliative Care

Job Summary:

We are seeking a skilled Social Worker to join our Palliative Care team. As a Social Worker for Palliative Medicine, you will play a vital role in supporting patients and their families through the advanced clinical practice of independent professional social work.

Key Responsibilities:

  • Coordinate and evaluate care to meet the psychosocial needs of palliative care patients and their families.
  • Provide direct care and case management services to ensure continuity of health care and maintain or enhance quality of life.
  • Collaborate with the Palliative Care team to advance the mission of service to patients and their families.

Requirements:

  • Licensed by the State of Iowa as a Licensed Independent Social Worker (LISW).
  • Master's degree in social work required.
  • Two years of experience with patients who have chronic illness or disease preferred.
  • Proof of completion of Mandatory Reporter abuse training specific to population served within three months of hire.
  • Demonstrates competence in providing case management services and conducting clinical interviews.
  • Ability to understand medical terminology and communicate effectively with persons of diverse backgrounds and ages.
