Licensed Independent Social Worker

MPAC Healthcare is seeking a Licensed Independent Social Worker to join our team. As a key member of our interdisciplinary team, you will provide high-quality care to older adults in skilled nursing facilities.
Key Responsibilities:
- Provide initial assessments and ongoing support to residents and their families
- Utilize psychotherapy to address mental health issues
- Collaborate with interdisciplinary teams to ensure residents receive comprehensive care
- Document patient information and maintain accurate records
Requirements:
- Master's degree in Social Work and valid LISW license
- Two years of counseling and therapy experience
- Demonstrated knowledge of Mental Health diagnoses and therapeutic approaches
- Experience collaborating with interdisciplinary teams
