We are seeking a dedicated and compassionate Home-Based Care Specialist to join our team at Sevita. As a Home-Based Care Specialist, you will play a vital role in providing high-quality care and support to individuals with intellectual and developmental disabilities in their own homes.

Key Responsibilities
  • Provide one-on-one care and support to individuals in their homes, promoting independence and community participation.
  • Assist with daily living skills, including meal preparation, medication administration, and personal care.
  • Support individuals in developing and achieving their goals, such as community participation, household budgeting, exercise, and nutrition.
  • Transport individuals to medical appointments, outings, and shopping trips, ensuring their safety and well-being.
  • Collaborate with our HCBS family to ensure the health and safety of the individuals we serve are being met in their family's homes.
Requirements
  • High School Diploma or GED
  • Minimum of 18 years of age
  • Valid driver's license in good standing
  • Access to a registered vehicle with proof of insurance
  • Six months of experience in human services preferred
