Job Summary:
We are seeking a highly skilled and experienced Care Management Specialist to join our team at Homewatch CareGivers. As a key member of our team, you will be responsible for providing exceptional care and support to our clients in their homes.
Key Responsibilities:
- Scheduling and coordinating care services for clients
- Conducting consultations with family members and clients to understand their needs and preferences
- Communicating effectively with clients, caregivers, and other team members to ensure seamless care delivery
- Working collaboratively with other Case Managers and team members to achieve shared goals
- Filing and documenting client information and care plans
Requirements:
- Experience in social work, case management, or a related field
- Excellent communication and interpersonal skills
- Ability to work independently and as part of a team
- Strong organizational and time management skills
- Ability to adapt to changing situations and priorities
