POSITION SUMMARY

The Housing Coordinator plays a vital role in connecting program participants and their families with appropriate housing solutions, fostering sustainable and healthy independent living. This position involves proactive community engagement to build resources that support individuals experiencing homelessness. As a key member of the service team, the Housing Coordinator offers leadership based on practical experience and engages in all phases of client support: assessment, service planning, engagement, and implementation.

KEY RESPONSIBILITIES

  • Identify and secure suitable housing options for clients and their families, including referrals to available shelters.
  • Establish and maintain a comprehensive housing database to connect tenants with available apartments.
  • Engage with landlords to inform them about the program and the specific needs of participants.
  • Lead community outreach initiatives, representing the organization at relevant meetings and building partnerships with local stakeholders.
  • Develop tailored support systems to address individual housing needs, including coordinating move-in logistics and rental negotiations.
  • Utilize community resources to assist tenants and landlords in resolving issues that may lead to eviction.
  • Collaborate with the case management team to provide information referrals, crisis intervention, and assistance with securing permanent housing.
  • Support case managers in conducting thorough assessments to gather essential information for developing Individual Service Plans.
  • Coordinate follow-up services in collaboration with the case management team.
  • Take on responsibilities of Case Manager or Housing Specialist as necessary.
  • Conduct outreach to the homeless population, coordinating efforts with community organizations to enhance service delivery.
  • Perform additional duties as assigned.

QUALIFICATIONS

REQUIREMENTS:

  • Successful completion of a fingerprint clearance and background check, including criminal history and employment verification.

EDUCATION:

  • Bachelor's Degree in Social Services or a related field, or
  • An equivalent combination of education and relevant work experience (four years).

EXPERIENCE:

  • A minimum of three years of experience working with veterans, individuals in recovery, or those experiencing homelessness.

PREFERRED QUALIFICATIONS:

  • Bilingual proficiency in Spanish.
  • Familiarity with HMIS (Homeless Management Information System).
