Job Description

We are seeking a dedicated Patient Care Coordinator to join our multidisciplinary team at the Community Psychiatric Institute. This is a unique opportunity to make a meaningful difference in the lives of individuals struggling with mental illness and substance abuse.


**About the Role**

  • This full-time or part-time position will be working Monday through Friday, 8:30am to 4:30pm, providing nursing and on-site health assessments, administering medications, coordinating medication refills and changes, facilitating medication education groups, and more.

**Responsibilities Include**

  • Working closely with our staff psychiatrist and Advanced Practice Nurse (APN)
  • Conducting thorough patient assessments to develop effective care plans
  • Communicating with pharmacies regarding medication refills and changes
  • Facilitating group sessions for medication education and counseling
  • Providing medical screenings as needed, consulting with medical providers to coordinate care
  • Maintaining accurate patient records, ensuring regulatory compliance with all state-mandated rules and guidelines

**Requirements**

  • Demonstrated experience in healthcare settings, preferably with a focus on psychiatric nursing
  • Strong communication and interpersonal skills, with ability to work effectively with diverse populations
  • Computer literacy and experience with Electronic Health Records (EHRs)
  • Ability to maintain confidentiality and handle sensitive information
