Community Care Specialist

We are seeking a highly skilled and compassionate Registered Nurse to join our team as a Community Care Specialist. This role will provide clinical and psychosocial care to individuals experiencing homelessness in community settings.
Responsibilities:
- Provide care to individuals at various locations, including encampments, interim housing/shelter sites, community health and resource fairs, and on the streets.
- Conduct patient assessments, document data through interviews, reviews of medical records, and analysis of diagnostic test results.
- Collaborate with providers, social workers, and other health professionals to develop effective care plans.
- Share responsibility for driving the Community Care Van to various service locations.
Requirements:
- Current, valid California RN license.
- Bachelor's or master's degree in nursing (BSN or MSN).
- BLS from the American Red Cross or American Heart Association.
- Current, valid California Driver's License - Class C.
- Proficiency in Word, PowerPoint, and Excel.
- Ability to share responsibility in driving the Community Care Van to various locations within the Los Angeles area.
Salary: $59.46/hour - $88.39/hour
