Position Overview

As a Home Care Intake Coordinator, you will play a vital role in connecting individuals with our exceptional home care services. You will be responsible for guiding potential clients through the enrollment process, ensuring a smooth and efficient experience from initial contact to service commencement.

Responsibilities

  • Collaborate with Community Outreach Representatives and potential clients to understand their needs and guide them through our comprehensive range of home care services.
  • Serve as the primary point of contact for referral sources, including Managed Long-term Care plans, Certified Home Health Agencies, and hospice agencies, to discuss member eligibility and initiate care processes.
  • Work closely with active patients to obtain necessary documentation and assist new patients in securing Medicaid coverage or connecting them with appropriate resources.
  • Maintain consistent communication with potential patients and referral sources to ensure a seamless intake and enrollment process.
  • Manage the referral intake process, from initial submission through admission status updates, ensuring accurate record-keeping and timely follow-up.
  • Analyze weekly status reports from various agency contacts and complete all required paperwork diligently.
  • Develop strong relationships with Program Managers, team members, and the Marketing team to facilitate effective communication and collaboration.
  • Gather clinical and demographic data from patients and sources, ensuring accuracy and completeness of information.

Qualifications

  • Fluency in Spanish is essential.
  • High School diploma or GED equivalent required.
  • Experience with Salesforce and HHA eXchange platforms is highly preferred.
  • Exceptional interpersonal skills, enabling you to effectively communicate with patients and build rapport.
  • A compassionate and patient-centered approach, dedicated to assisting individuals in accessing home care services that meet their needs.
  • Familiarity with home care procedures or experience with HHA eXchange is advantageous.
  • Ability to thrive in a collaborative team environment.
