Job Title: Residential Case Manager

Veterans Community Project (VCP) is seeking a highly motivated and compassionate Residential Case Manager to join our team. As a Residential Case Manager, you will be responsible for providing supportive housing services to homeless Veterans and their families, formulating case plans that promote Veterans moving towards permanent housing readiness, and working collaboratively with all programs and agency staff to facilitate a team environment.

Key Responsibilities:
  • Intake Process: Completes initial interviews and background checks for potential residents, completes an initial needs assessment, and orients clients to VCP programs, requirements, and expectations.
  • Case Planning: Develops an individualized client-driven case plan with both short-term and long-term goals, monitors progress towards goals in regularly scheduled weekly sessions, evaluates and adjusts case plans as needed, and provides written warnings with consequences if satisfactory progress is not being met.
  • Reporting: Maintains client files to include conversations, warnings, progress towards goals, and documentation of any incidents, reports critical incidents immediately to Veterans Support Services Director, collects data necessary to meet funding requirements and statistical reports, and completes daily housing log, daily conversation, and case management logs, submitting them to VCP's Customer Relationship Management (CRM) software.
  • Property Management: Assures the safety of each property through frequent tours of the residential area inspecting for any hazards or repair needs, reports any hazards to Operations for immediate attention, and facilitates the repair process in collaboration with Operations.
Requirements:
  • Bachelor's degree required.
  • Two years experience providing direct care to individuals in a social service setting.
  • Veteran or strong Veteran-based experience.
  • Excellent oral and written communication skills.
  • Desire to continuously learn and grow.
