Key Responsibilities:

As a Licensed Practical Nurse (LPN), your duties will include:

  • Delivering in-home direct patient care as per the prescribed care plan while monitoring patient progress and recovery.
  • Assessing and documenting patient conditions, reporting changes as necessary (including vital signs and other health indicators).
  • Administering medications, insulin, and IV fluids with thorough documentation.
  • Inspecting and caring for wounds, changing dressings, and assisting with personal hygiene.
  • Evaluating patients for additional injuries, infections, or bedsores, and providing appropriate treatments.
  • Collaborating with healthcare providers, aides, and allied health professionals to ensure optimal patient outcomes.

Qualifications:

To be considered for the Licensed Practical Nurse (LPN) position, you must possess:

  • A current unrestricted license to practice as a Licensed Practical Nurse (LPN) in the relevant state.
  • Current CPR/AED/BLS/First Aid certification.
  • Reliable transportation to and from care sites.
  • Experience with tracheostomy and/or ventilator care is preferred but not mandatory.
