Patient Care Manager

We are seeking a dedicated and compassionate Patient Care Manager to join our team at Three Oaks Hospice. As a key member of our interdisciplinary team, you will play a vital role in ensuring the highest-quality care for our patients and their families.
Key Responsibilities:
- Coordinate and supervise an interdisciplinary team of staff to deliver exceptional patient care.
- Develop and implement individualized care plans in accordance with physician-prescribed plans of care.
- Ensure compliance with state and federal laws and regulations.
- Collaborate with healthcare professionals to provide comprehensive care services.
- Provide leadership and guidance to team members to promote a culture of excellence.
Requirements:
- College degree; Registered Nurse/BSN preferred.
- 2+ years of hospice experience and supervisory experience required.
- Knowledgeable of hospice regulations.
- Proficient in using a computer and Microsoft Office suite.
- Excellent communication and interpersonal skills.
