About the Role

We are seeking a skilled Registered Nurse to join our Care Team as a Care Manager RN. As a key member of our integrated team, you will work closely with patients, physicians, and other healthcare professionals to provide high-quality care and improve health outcomes.

Key Responsibilities
  • Develop and implement personalized care plans for patients with chronic kidney disease and end-stage renal disease
  • Conduct in-home visits to assess patient needs and provide education on disease management and treatment options
  • Collaborate with patients' medical providers to ensure seamless transitions of care and coordinate with dialysis providers as needed
  • Monitor patient progress and adjust care plans as necessary to ensure optimal health outcomes
  • Provide education and coaching to patients and their families on disease management, medication adherence, and lifestyle choices
  • Engage with patients and their support networks to promote health equity and address social determinants of health
Requirements
  • Graduate of an accredited School of Nursing with a current RN license in the state of employment
  • 2+ years of experience in care management and/or working with patients with chronic kidney disease and end-stage renal disease
  • Excellent communication and interpersonal skills, with the ability to build strong relationships with patients and healthcare professionals
  • Ability to work independently with minimal supervision and take call remotely on some nights and weekends
