Key Responsibilities- Develop and implement personalized care plans for patients with chronic kidney disease and end-stage renal disease
- Conduct in-home visits to assess patient needs and provide education on disease management and treatment options
- Collaborate with patients' medical providers to ensure seamless transitions of care and coordinate with dialysis providers as needed
- Monitor patient progress and adjust care plans as necessary to ensure optimal health outcomes
- Provide education and coaching to patients and their families on disease management, medication adherence, and lifestyle choices
- Engage with patients and their support networks to promote health equity and address social determinants of health
- Graduate of an accredited School of Nursing with a current RN license in the state of employment
- 2+ years of experience in care management and/or working with patients with chronic kidney disease and end-stage renal disease
- Excellent communication and interpersonal skills, with the ability to build strong relationships with patients and healthcare professionals
- Ability to work independently with minimal supervision and take call remotely on some nights and weekends
