Kaiser Permanente is seeking a skilled Social Work Specialist to join our team in a dynamic and challenging role.
Key Responsibilities- Participate as a member of the emergency department care team, providing social medicine expertise in response to care team referrals and case finding of high need populations.
- Conduct psychosocial assessments of patients in the emergency department, identifying psychosocial needs, developing, and implementing plans of care.
- Assist patients and their support systems with coping strategies related to illness, providing information and referrals to community resources for post-discharge support.
- Conduct social medicine consultations and assistance for cases involving abuse, neglect, advance care planning, and legal decision making.
- Contribute to the patient discharge planning process, assessing patient and support system choices and educating them on the risks and benefits of their choices to facilitate discharge.
- Provide information, resource identification, and referral services, mediation services, advocacy services, and education to patients and their support systems.
- Master's degree in Social Work.
- Minimum one year of experience in a healthcare setting.
- Licensed Clinical Social Worker (Hawaii) or Licensed Social Worker (Hawaii).
- Basic Life Support certification from the American Heart Association.
- Experience with interdisciplinary team function.
- Knowledge of medical terminology, the psychosocial impact of serious illnesses.
- Demonstrated ability to interrelate with physicians, nurses, support staff, and patients.
