Key Responsibilities:
  • Conduct in-home assessments, including medical history evaluation, system review, medication reconciliation, vital signs, and comprehensive physical exams.
  • Perform evidence-based practice screenings, including point-of-care testing (as appropriate).
  • Identify diagnoses for care management and active medical management in furtherance of treatment.
  • Formulate a list of current and past medical conditions using clinical knowledge and judgment, along with assessment findings.
  • Communicate assessment results to inform members' primary care physicians of potential care gaps.
  • Recognize urgent and emergent situations and intervene accordingly.
  • Educate members on disease processes, medication, and compliance.
Requirements:
  • Active NP license or ability to obtain by start date, with unencumbered licensure free of any open/unresolved disciplinary actions.
  • APCs working in jurisdictions authorizing autonomous practice must have obtained approval to practice autonomously or without formal supervision from their licensing board, if applicable.
  • Active ANCC or AANP national certification in Family, Geriatrics, Adult, Adult-Gerontology Primary Care, or Adult-Gerontology Acute Care certification, or ability to obtain national certification and/or NP license in state of assignment by start date.
  • Ability to complete physical requirements of the job, including lifting a 30-pound bag, navigating stairs, and various dwelling conditions.
  • Reliable transportation for home visit assessments.
Preferred Qualifications:
  • 1+ years of clinical experience in the highest level of education, clinical setting.
  • May be requested to obtain additional licensure in other geographic areas.
  • Experience in gerontology, cardiology, internal medicine, or endocrinology.
  • Home Health care or home visit experience.
  • Ability to travel throughout the state, beyond assigned regions, up to 5% of the time, with advanced notice and potential for additional compensation incentives.
  • Fluency in Spanish, Cantonese, Korean, Vietnamese, Polish, or other languages.
Compensation and Benefits:

UnitedHealth Group offers a comprehensive benefits package, including a salary range of $88,000 to $173,200 annually, based on local labor markets, education, work experience, certifications, and other factors. Additionally, you'll have access to incentive and recognition programs, equity stock purchase, and 401k contribution, subject to eligibility requirements.
