Job Summary

We are seeking a skilled Hospice Nurse Practitioner to join our team on a per diem basis. As a key member of our interdisciplinary team, you will be responsible for conducting Face-to-Face (F2F) encounters, gathering clinical information, and communicating findings to our hospice physicians.

Responsibilities
  • Conduct F2F encounters and gather clinical information to support hospice diagnosis and prognosis
  • Communicate findings to hospice physicians for certification or recertification
  • Perform physical assessments within regulatory time frames
  • Provide clinical findings and recommendations to the IDG
  • Notify the hospice agency of any scheduling issues
Requirements
  • Master's or higher degree in Nursing
  • Graduate-level nurse practitioner education program accreditation
  • Current state license as a registered nurse
  • Current state license or certificate as an advanced practice nurse
  • Current national certification as a Nurse Practitioner
  • Collaborative agreement in place
