Job Summary

We are seeking a skilled Clinical Resource Liaison to join our team at PACE Southeast Michigan. The successful candidate will provide exceptional support to our nursing team, ensuring seamless communication and coordination of care for our participants.

Key Responsibilities
  • Facilitate effective communication through phone calls, virtual meetings, and written correspondence to assist the nursing team and participants.
  • Coordinate urgent calls and tasks to ensure timely resolution and participant satisfaction.
  • Collect and prepare specimens for laboratory testing, and deliver essential supplies to participants.
  • Assist with procedures, vital signs, and other related duties as assigned.
  • Support the facilitation of virtual visits and wellness calls to enhance participant engagement and care.
Requirements
  • Excellent customer service and communication skills, with the ability to navigate electronic medical records and participant-oriented software.
  • Organization and time management skills to prioritize tasks and meet deadlines.
  • Familiarity with medical terminology and the ability to recognize urgent medical issues.
  • Experience in a healthcare setting, preferably as a PCA, CNA, nursing student, MA, or EMT.
  • Proficiency in Microsoft Office and ability to learn new software applications.
Working Conditions
  • Fast-paced and flexible environment with possible exposure to communicable diseases.
  • Able to lift up to 50 pounds and possess dependable transportation and a valid driver's license.
  • This role requires a combination of center-based and remote work, with occasional in-home, clinic, and office visits.
