Job Summary

The Clinical Resource Liaison plays a vital role in supporting the delivery and coordination of care to participants at PACE Southeast Michigan. This position works under the direction and supervision of the Registered Nurse Case Manager (RNCM) and is responsible for facilitating communication, coordinating care, and providing administrative support to the nursing team.

Key Responsibilities
  • Communication and Coordination: Facilitate communication through answering direct nursing phone calls to assist with nonclinical, non-urgent matters, and direct calls to appropriate team members. Serve as a liaison between the nursing team and participants/caregivers.
  • Urgent Call Facilitation: Facilitate urgent calls to appropriate team members in a timely and efficient manner.
  • Specimen Collection and Delivery: Collect and prepare urine and stool specimens for testing.
  • Supply Delivery: Deliver urgent supplies, such as diabetic and incontinence products, as needed.
  • Administrative Support: Assist with other related duties as assigned, including vital signs, assisting nurses with procedures, and facilitating virtual visits for participants.
  • Wellness Calls and Appointment Reminders: Make wellness calls and send appointment reminders to participants as needed.
  • Record Retrieval and Faxes: Assist with retrieving records and sending faxes as required.
Requirements and Qualifications
  • Excellent Communication and Customer Service Skills: Possess excellent face-to-face, telephone, virtual, and written communication skills.
  • Organization and Time Management: Demonstrate strong organization and time management skills to prioritize tasks and meet deadlines.
  • Medical Terminology and Urgent Medical Issues: Familiarity with medical terminology and the ability to recognize urgent medical issues.
  • Experience and Education: Previous experience as a PCA, CNA, nursing student, MA, or EMT, or equivalent education and experience.
  • Technical Skills: Proficiency in Microsoft Office and ability to navigate electronic medical records and other participant-oriented software.
  • Flexibility and Adaptability: Ability to flex to the needs of participants and the center, as required.
Working Conditions
  • Fast-Paced and Flexible Environment: Work in a fast-paced and flexible environment with possible exposure to communicable diseases.
  • Lifting and Transportation: Ability to lift up to 50 pounds and possess dependable transportation and a valid driver's license.
  • Remote Work: This position may require some in-home, clinic, day health center, office, and remote work.
